Aesthetic Case Check List

The following are essential tools we will need to complete an aesthetic case. These should be obtained during the prep appointment and/or temp check appointment:

1. All 6 requested photos (see Photo Guide Below).

2. Model of the pre-op.

3. Impressions of the temporaries
   (gingival capture is as important as capturing the margins).

4. Bite registration.

5. Stick-bite (horizontal plane, parallel to horizon line between pupils).

6. Detailed lab prescription, thoroughly filled out.

Diagnostic Wax-Up Check List

The following are essential tools we will need to design your aesthetic case. These should be obtained during the initial diagnostic appointment and prior to prepping:

1. All 5 requested photos listed (see Photo Guide attached)

2. Full arch impressions of both upper and lower arches (must include good gingival capture)

3. Bite registration, including CR/CO bites

4. Stick-bite (horizontal plane, parallel to horizon line between pupils)

5. Detailed lab prescription to include:

   • A clear expression of the patient’s goals

   • The measurement of desired final length of the centrals

   • New vertical measurements from CEJ to CEJ
